Tennova Medical Group Adds Patient Care Providers

CLARKSVILLE, TN (April 3, 2025) – Tennova Medical Group welcomes Certified Nurse Practitioners Leslea Streetman and Jane Holt. Ms. Streetman is now accepting patients at Tennova Family Medicine - Tiny Town, 2485 Tiny Town Road in Clarksville. Ms. Holt will be serving the patients of Tennova Surgery - Clarksville, 647 Dunlop Lane in Clarksville.

Leslea Streetman, AANP-C, is a certified family nurse practitioner serving the patients of Tennova Family Medicine - Tiny Town. She received both her bachelor’s and master’s degrees in nursing from Austin Peay State University in Clarksville. She also holds a BS in Corporate Communications from APSU. Leslea has experience in patient care clinics and hospital settings. Beginning her career as a nurse in women’s services, she has also worked in pain management, weight loss solutions, and urgent care. Leslea has served as an instructor for area MA programs.

Leslea is a member of the American Association of Nurse Practitioners, the Tennessee Nurses Association, the TN Pain society, and is a Certified Legal Nurse Consultant. When she is not at work, the Clarksville native enjoys spending time with her family. Leslea offers prevention and management of chronic conditions, well checks, sick visits and routine health screenings for patients ages 4 and up.

Jane Holt, AGNP-C is a certified nurse practitioner serving the patients of Tennova Surgery - Clarksville. She received her bachelor's degree in nursing from the University of Tennessee in Knoxville and her Master of Science with a concentration on nurse practitioner from St. Louis University in St. Louis, Missouri. Jane, a native of Clarksville, served as a nurse for many years inside the Clarksville hospital, working in both oncology and critical care. Jane then moved to Tennova Cardiology - Clarksville, serving through 2020, where she cared for many patients, both in the clinic and during their hospital stays. She is now working directly with Dr. Ronald Atwater at Tennova Surgery - Clarksville and will assist with patients during the recovery process.

Jane is a member of the American Association of Nurse Practitioners, the Tennessee Nurses Association, and the American College of Cardiology. When she is not at work, she enjoys spending time with her family, especially her daughters. Tennova welcomes Jane back to our organization.
